We will collect, use, store, share or otherwise Process personal data about you (or persons connected to you) including:

- Date of birth
- Contact details such as your email address, physical address and telephone number and
- Medical records available in physical files and electronic form
- Any other information required for the purpose of providing medical care to you including, but not limited to, your medical insurance details.
Purpose of Data Collection:
- Providing Medical services
- Communication - to contact, send appropriate healthcare and educational materials either in, or via calls, SMS (Short Message Service) - and email form and/or portal.
- Referral lab - If part of the treatment requires lab tests as part of the diagnosis process, we will need to carry out the tests at labs that provide those specified tests.
- Referral Hospital - In the event that my treatment requires admission / specialist review.
- Insurance companies – Where the service rendered is payable by an underwriter documentation is required as proof to service delivery and for quality control processes.
Data Processing
Your data will be processed for the purposes outlined above and may involve storage, analysis, and sharing with third parties, where necessary, to provide utmost quality care to you and to fulfill the obligations set out under the Data Protection Act.

We will retain your personal data for as long as necessary to fulfill the purposes outlined in this form, or as required by law.

You have the right to be informed of the use to which the personal data is to be put, access your personal data in our custody, object to the processing of all or part of the data, rectify/correct of false or misleading information, or request the deletion of your personal data held by us.
